Why this family was referred: the flagged finding, the screening band, and the athlete’s own words where captured. No diagnosis — a documented reason for professional review.

History & symptoms

Injury history, current symptom screen (pain, swelling, instability, locking), sport, position, and exposure context — collected with guardian consent, structured for a clinical intake.

Movement findings

Task-by-task results with the trials behind them: which attempts were valid, which were rejected and why, and the quality checks each metric passed before it was allowed to count.

Strength & capacity context

Objective measures where available, with device, protocol, and units preserved — plus interlimb comparisons framed as screening signals, not cut-points.

Trend & change

This assessment against the athlete’s prior ones. Change is only called change when it exceeds measurement error — otherwise the packet says so plainly.

Provenance appendix

Protocol version, algorithm version, device identifiers, assessor, and date for every number above. If you want to know how a value was produced, the answer is in the packet.

Every metric traces to a specific trial, protocol version, and algorithm version. Nothing arrives without its provenance.

Honest about uncertainty

Invalid trials are excluded and labeled. Low-confidence measures are marked. “No confident change” is a result we actually report.

The packet supports your evaluation; it does not pre-empt it. No diagnosis, no treatment directive, no return-to-play call.
